Centura Health at Home Foundation, ColoradoThe Centura Health at Home Foundation proudly supports home health services and senior living communities offered by Centura Health at Home across Colorado. Centura Health at Home brings a continuum of expert, innovative, compassionate care to people of all ages, in all the places they call home. Our continuum of care includes home care, telehealth services, rehabilitation therapies, palliative care, both inpatient and home hospice services and senior living communities offering independent living, assisted living, nursing home care, memory care and adult day programs.

The Centura Health at Home Foundation raises funds to support residents at Centura Health's senior living communities and patients and families needing home health care and supportive services. As a non-profit organization, the Centura Health at Home Foundation relies heavily on community support. With your help, we can continue to offer specialized programs and provide care for those who are underinsured or can not afford home health or senior living services.

The 9th annual Hike for Hospice will be held on Sunday, September 14, 2014 in support of Porter Hospice and St. Anthony Hospice. Hikers will enjoy beautiful views as they climb 7,600 feet to the summitt of Olinger Mount Lindo, home of the illuminated cross in Morrison, Colorado. Participants can hike in the name of loved ones and honor them by doing a memorial release at the summit while their loved one's name is read. Register today!

All funds raised at Hike for Hospice go towards supporting charity care and hospice services including spiritual care, bereavement services, volunteer coordination and palliative care for children through the Butterfly Program
